- The new paradigm
- Diagnosis: In the past, the AMA Guides emphasized functional assessment, where impairment ratings could be based on loss of strength, loss of range-of-motion, or loss of sensation. Now, the AMA Guides emphasize diagnosis, where impairment rating can be based on whether the injury is to soft tissue, muscle or tendon, or ligament, bone, or joint.
- Adjustment: The diagnosis-based impairment is then adjusted up or down according to other non-key factors, such as functional history, physical examination, and clinical studies.
- Diagnostic-based grids: Diagnostic-based grids were developed for each organ system. The grids arrange diagnoses into five classes of impairment severity, according to consensus-based dominant criterion.
- Severity grid: The functionally based history, physical findings, and broadly accepted clinical test results, where applicable, are then integrated to determine severity grade and provide a corresponding impairment value.
- Examples of conditions for which impairment rating will be lower under Sixth Edition:
- Two-level cervical fusion
- Carpal tunnel syndrome
- Notable changes regarding the assignment of impairment ratings in various categories of injuries
- Shoulders
- Neck/cervical spine
- Upper extremities
- Lower extremities
- Lower back
